Who Are Health TAPESTRY Clients

Health TAPESTRY is identifying people at risk before a person needs invasive and expensive interventions, like going to a hospital. We work with these people and their communities to help them be healthier, where they live, for longer.

Our sites across Canada are working with:

Older Adults (70 years and older): Older adults are more likely than most people to have one or more chronic health conditions. They tend to use the health care more and they are likely to benefit from working with Health TAPESTRY.

People living with chronic conditions: The Health TAPESTRY approach can help people self-manage their health care needs. People with chronic conditions, like diabetes and hypertension, may benefit from our online tools and the help of volunteer Health Connectors. Health TAPESTRY can improve a client’s motivation and help them access the health system and community resources.

Other target populations: Health TAPESTRY is also working with people with complex medical needs, frequent users of the health system, people recently discharged from hospital, newcomers that have not used the health system, First Nations communities, homebound frail elderly people and other groups that may benefit from our approach.
