The Health TAPESTRY intervention is based on four<\/strong> main parts:<\/p>\n\n\n\n

    \n
  1. Interprofessional primary health care<\/a> teams that provide health care services to the Health TAPESTRY client and work with them on meeting their goals,<\/li>\n\n\n\n
  2. Trained community volunteers<\/a> that connect with clients in their home to gather health and social information and to discuss life and health goals,<\/li>\n\n\n\n
  3. Community engagement and connections<\/a> that helps the Health TAPESTRY approach fit to the changing needs of a community by learning from the community and forming partnerships with other organizations, and<\/li>\n\n\n\n
  4. The use of technology<\/a> that connects clients, their health care team, volunteers and the community.<\/li>\n<\/ol>\n\n\n\n

    The four parts of Health TAPESTRY each play a role in a process called system navigation. System navigation helps a client understand and access a strong network of community organizations, including their primary care team, as part of their health care.<\/p>\n\n\n\n
