{"id":12,"date":"2025-05-14T13:29:00","date_gmt":"2025-05-14T13:29:00","guid":{"rendered":"https:\/\/healthtapestry.com\/?page_id=12"},"modified":"2025-05-14T13:29:01","modified_gmt":"2025-05-14T13:29:01","slug":"the-health-tapestry-intervention","status":"publish","type":"page","link":"https:\/\/healthtapestry.ca\/index.php\/what-is-health-tapestry\/about-health-tapestry\/the-health-tapestry-intervention\/","title":{"rendered":"The Health TAPESTRY Intervention"},"content":{"rendered":"\n<p style=\"font-size:1rem;font-style:normal;font-weight:400\">The Health TAPESTRY intervention is based on <strong>four<\/strong> main parts:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><a href=\"https:\/\/healthtapestry.ca\/index.php\/the-four-parts\/interprofessional-health-teams\/\">Interprofessional primary health care<\/a> teams&nbsp;that provide health care services to the Health TAPESTRY client and work with them on meeting their goals,<\/li>\n\n\n\n<li><a href=\"https:\/\/healthtapestry.ca\/index.php\/the-four-parts\/trained-volunteers\/\">Trained community volunteers<\/a>&nbsp;that connect with clients in their home to gather health and social information and to discuss life and health goals,<\/li>\n\n\n\n<li><a href=\"https:\/\/healthtapestry.ca\/index.php\/the-four-parts\/community-engagement-and-connections\/\">Community engagement and connections<\/a>&nbsp;that helps the Health TAPESTRY approach fit to the changing needs of a community by learning from the community and forming partnerships with other organizations, and<\/li>\n\n\n\n<li>The use of&nbsp;<a href=\"https:\/\/healthtapestry.ca\/index.php\/the-four-parts\/technology\/\">technology<\/a>&nbsp;that connects clients, their health care team, volunteers and the community.<\/li>\n<\/ol>\n\n\n\n<p style=\"font-size:1rem;font-style:normal;font-weight:400\">The four parts of Health TAPESTRY each play a role in a process called system navigation. System navigation helps a client understand and access a strong network of community organizations, including their primary care team, as part of their health care.<\/p>\n\n\n\n<p class=\"has-nv-dark-bg-color has-text-color has-link-color wp-elements-d4e80430b5dae24cf8ac74e044195442\" style=\"font-size:30px;font-style:normal;font-weight:700\">Research Findings<\/p>\n\n\n\n<p style=\"font-size:1rem;font-style:normal;font-weight:400\">Below you will find summaries of research studies evaluating the Health TAPESTRY intervention as a whole.
