Complex programs involve investment of time and resources by both patients and the health care system, so it is important to study them to understand what they achieve for patients and how they may be improved.<\/p>\n\n\n\n<p>In the Health TAPESTRY program, two trained community volunteers visit each Health TAPESTRY client in their home. They ask about clients\u2019 health needs and goals to understand what matters most to them. The volunteers enter the client responses into the TAP-App (a custom-built application) using a tablet. Their responses are automatically summarized in a TAP-Report and sent to the client\u2019s usual primary care team where a group of primary care providers (the TAP-Huddle) views the report, discusses each clients\u2019 needs and goals, and creates plans of care. The TAP-Huddle is made up of interprofessional care providers (e.g., pharmacist, occupational therapist, dietitian, etc.).
