In Health TAPESTRY, the interprofessional health care team is the group of registered professionals that work together to provide primary care to the client. The Health TAPESTRY approach may involve a diverse health care team, including:
- family physicians,
- nurses,
- occupational therapists,
- physiotherapists,
- dietitians,
- social workers,
- pharmacists and
- other health care professionals.
When an interprofessional health care team is part of Health TAPESTRY, it gives the team members an opportunity to reflect on their roles in caring for clients in a new way. It can also help them better understand how their work and their client’s health needs are affected by the other three parts of the Health TAPESTRY approach (volunteers, community engagement, and technology).

The implementation and validation of the NoMAD during a complex primary care intervention
Why did we do the study? The NoMAD (Normalization Measurement Development) is a 23-item tool that measures implementation. It is based on Normalization Process Theory. We used the NoMAD to understand how Health TAPESTRY became “normalized” into everyday clinical practice. Because it is a relatively new tool, in this we…
Understanding how Context and Culture in Six Communities can Shape Implementation of a Complex Intervention: A Comparative Case Study
Why did we do the study? Health TAPESTRY was implemented in six communities across Ontario, Canada. Each community differed in many aspects (e.g., size, rural vs. urban, etc). The primary care teams involved in Health TAPESTRY were also vastly different (e.g., number and type of providers, in-clinic programs, etc). We…
Examining interprofessional teams structures and processes in the implementation of a primary care intervention (Health TAPESTRY) for older adults using normalization process theory
Abstract Background Many countries are engaged in primary care reforms to support older adults who are living longer in the community. Health Teams Advancing Patient Experience: Strengthening Quality [Health TAPESTRY] is a primary care intervention aimed at supporting older adults that involves trained volunteers, interprofessional teams, technology, and system navigation.…
